When going on LOA it is important that you complete the LOA form ahead of time and then complete the departure and return forms when you leave and return. We need this information for planing purposes and because we are required to submit occupancy filings to the state of West Virginia. We rely on Purusha submitting a LOA form when departing and upon return so that we can accurately fill out the WV occupancy report.
The arrival and departure forms should also be filled out by those going to or arriving back from India or other Purusha locations.
If you need to go on an extended LOA (more than 30 days per year) fill out the Purusha Extended LOA Application and contact Steve, Rick, or Jeff.

LOA Notification Form: Fill out sometime prior to actual departure with estimated dates of LOA.
LOA Actual Departure Date Form: Fill out when you are sure of your actual departure date.
LOA Actual Return Date Form: Fill out when you return.
Extended LOA Form: Fill out if you need to go on an extended LOA (over 30 days per year).
